Report Your Hours

Thank you for sharing your time and talents! 
Please log your volunteer hours and submit them to SRI Volunteers monthly.  This will help SRIV to:

• Ensure continued "programming and opportunities"
• Assess community needs and responses
  
• Demonstrate to supporters the value of the services you provide
  
• Support "you" our SRIV Volunteers and your vital contributions to the community at large

Please make sure you fill in your NAME and select the appropriate MONTH before you send this form.  Call us immediately if you get a new volunteer job. If you volunteer at more than one site, use a separate report for each. Each volunteer assignment you accept is important to our community as it highlights a need that is being addressed.


NOTE: If you deliver Meals on Wheels for South Kingstown and Narragansett and you use the sign-in sheets at the office, it is not necessary to report hours using this on-line form.
